Level 2 Social Worker

We are offering an exciting opportunity for a Level 2 Social Worker to join our ambitious, innovative and supportive Adult Social Care Community teams based in Codsall, South Staffordshire.Youwill be a highly skilled Registered Social Worker and play a key role in supporting the delivery of the Personalisation and inclusion Pathways for Adults and Carers ensuring legal literacy, Strength and asset-based practice, personalisation and social inclusion is embedded and applied effectively throughout their work.The service predominately supports adults aged 18+ with physical disabilities and older people over the age of 65 with physical and/or mental health needs and will be expected to demonstrate a sound working knowledge of the Care Act, Mental Capacity Act and other legal and statutory frameworks.You will also be required to hold either Approved Mental Health Professional, Best Interest Assessor or Practice Educator qualification or the willingness to work towards obtaining one of these.This role is a permanent, full-time position although consideration will be given to applicants wanting to work on a part-time basis. We also offer a hybrid blended working approach of home and office. We also offer the option of working a 9-day fortnight to full time workers.Main duties of the jobYou will be advocating and working with adults with complex needs and their carers, working collaboratively with colleagues from other agencies to develop support and services appropriate to people’s individual needs utilising a strength-based approach.As part of this role, you will need to be:Confident carrying out outcome-focused assessments, with support planning, to meet their care and support needs.Provide more comprehensive/experienced social work support to individual adults and their carers and families, and involve them in all planning and actions taken, helping to signpost/refer on (where appropriate) and/or develop social resilience, motivation and daily living skills.Participating on the Duty/Access and priority rotas, to carry out a range of ‘first point of contact and priority’ functions including screening requests for assessments for health and social care services for adults and carers via Duty/Access utilising a range of assessment tools including Risk Screening Tools.Where appropriate, initiate an assessment including proportionate assessments and multi-disciplinary assessments.
